Notes for New Patients
Thank you for choosing us to be part of your healing process. We appreciate that you have entrusted us with your care, and we are confident that acupuncture and Oriental medicine will help restore you to a state of health and balance. As you may know, there is a considerable body of research supporting the effectiveness of acupuncture for the treatment of a wide range of medical, emotional and quality of life issues. If we can help you research your condition, please let us know.
Things you should complete and bring with you:
The "New Patient Health History," below, is the usual type of health history form you would find in any healthcare office. It is important that you fill out this form and bring it with you to your first treatment.

Things to do before your acupuncture appointment:
Wear or bring loose clothing, such as shorts, sweatpants, and a t-shirt or bikini top. Street clothes restrict the body and may prevent access to important acupuncture points, thus reducing the effectiveness of your treatment.
Try not to eat a large meal right before an acupuncture treatment, but don't come hungry either, as either will negatively affect the treatment.
Your pulse plays a large part in Oriental medicine. Coffee, alcohol and cigarettes strongly affect the pulse and make it more difficult to make a complete diagnosis. If possible, please do not consume these substances for at least two hours before a treatment.
